Can I fax my immunization records to GPC?
Answer
Yes.

Immunization Records can be faxed to 678-891-3211. Please note this is for immunization records only.

Be sure that your GPC-ID is on the form when you fax it. If the form is not legible, you will be asked to re-submit the form.

All other records should be faxed to 678-891-3280.
