Applicants who have answered 'Yes' to any of the questions on the admission application regarding criminal or disciplinary charges or convictions will be required to complete a questionnaire that will provide additional information that will be used to make an admission decision.
This form will be mailed to the applicant or may be obtained in any Enrollment and Registration Services office. |
